Purpose: The demand for vascular surgeons is increasing in Australia due to the burden of chronic disease and an ageing population. Most vascular surgeons work in major metropolitan centres, creating disparity and inequality in the care provided to patients in rural areas. This paper aims to review the rural vascular surgery workforce and identify potential avenues to improve patient access to vascular surgeons in rural areas. Methodology: A review of workforce, population and projection reports from the Royal Australian College of Surgeons, the Australian New Zealand Society for Vascular Surgery, and the Australian Institute of Health and Welfare was completed. Papers were reviewed, and information was collated to analyse the rural and remote Australian vascular surgery workforce. Results: In 2022, there were 274 registered vascular surgeons in Australia, servicing a population of 26,124,814. Vascular surgeons represent the second smallest cohort of specialist surgeons and have the 5th lowest representation in rural areas compared to the other surgical specialties. Less than 10% of Australian vascular surgeons work in rural and remote regions, which comprise 28% of Australia’s population. In 2019, 27.3% of vascular surgeons based in metropolitan centres provided outreach to rural and remote areas. Conclusion: Patients in rural areas have higher rates of peripheral vascular disease and experience poorer outcomes compared to their metropolitan counterparts. Vascular surgery will likely experience increased demand due to the growing prevalence of comorbidities and an ageing population. There is a maldistribution of vascular surgeons and an underserving of rural and remote areas. Addressing this complex issue will require comprehensive and innovative strategies. The United States of America has utilised many different policies to promote rural healthcare, and some of these may be able to be implemented in Australia.
