Paediatric vascular trauma cases necessitate swift and precise interventions, often requiring tailored vascular grafts for effective management. This study introduces a standardized approach for constructing paediatric vascular grafts ex situ using bovine pericardium, suitable for emergent open trauma cases. Our primary objective is to outline a robust guide and equipment list for creating these grafts, applicable to contemporary vascular centres. The methodology involves selecting bovine pericardium of various sizes and fashioning them into tubular grafts over arterial sheaths to ensure standardized diameters. Intravascular ultrasound will be employed to validate assumed in situ diameters, ensuring graft compatibility with paediatric vasculature. A quick-reference guide summarizing graft sizes and specifications will be tabulated, facilitating rapid selection and deployment during emergency procedures. Preliminary results demonstrate the feasibility and reproducibility of this back table construction method, with anticipated benefits including expedited graft preparation and enhanced intraoperative efficiency. Challenges such as optimal sizing and material handling will be discussed, alongside strategies for mitigating procedural complexities. By presenting a detailed equipment list and rationale for each component, this study aims to establish a practical framework for paediatric vascular graft construction in emergency settings. The outlined approach not only addresses the critical need for immediate vascular access but also promotes standardized practices across diverse healthcare environments. In conclusion, our study advocates for the adoption of back table construction techniques in paediatric vascular surgery, offering a systematic approach to enhance procedural readiness and optimize patient outcomes in emergency trauma scenarios.
